IRS Form 1095-C (Employer-Provided Health Insurance Offer and Coverage) to current and former employees upon request.

How to Request a Copy of Your Form 1095-C

Requests may be made by current or former employees. Once requested, Form 1095-C will be furnished within 30 days of the request.
